Attendees are allowed to bring one guest at a cost of $50 USD. Please enter the guest's name at the right and then PRINT this screen BEFORE pressing 'Submit' below. Then mail the printed copy with the $50 USD to: 2008 Leadership Summit, HFTP, 11709 Boulder Lane, Ste 110, Austin, TX, 78726.  

Chapters will receive an automatic (1) $1000 stipend for Leadership Summit attendance. Attendance counts after the evaluation form is filled out at the end of day two of the conference. Leadership Summit stipends are allocated to chapters in November.

PLEASE NOTE, it is the chapter's responsibility to reimburse the attendee(s) for travel expenses. Additional stipends are no longer granted.
